I ran across some easy code to implement on your dreamweaver site. I use CS5. Go to this link and follow the directions closely – http://www.freecontactform.com/email_form.php . Also, make sure you also edit the PHP settings in .htaccess file located in your domain root and put custom sendmail_from:
